In the early 1950s, as reports of “flying saucers” reached Canadian military, police and government offices, officials recognised a basic problem: different departments collected different kinds of information, making reports difficult to compare. What information the form required
The proposed reporting form reflected Peter Millman’s view that reliable investigation depended on measurable observations rather than dramatic stories. Officials wanted reports to include information such as:
- the exact date and time of the observation;
- the location of both the witness and the object;
- the object’s apparent direction of travel;
- estimated altitude, speed and angular size;
- weather conditions and visibility;
- the duration of the sighting;
- the number of witnesses;
- whether binoculars, cameras or other instruments were used;
- sketches of the object’s appearance and movement; and

The emphasis on structured observations mirrored scientific field reporting rather than intelligence questionnaires. Officials hoped that consistent measurements would make it easier to distinguish astronomical objects, meteors, balloons, aircraft and genuine unknowns.
How officials expected reports to be compared
Project Second Storey’s reporting form was designed to solve a practical administrative problem. During the early Cold War, reports could arrive through the Royal Canadian Mounted Police, the Department of National Defence, the Department of Transport, local police forces or civilian correspondence. The committee expected each department to collect essentially the same information so that analysts in Ottawa could:
- compare reports from different regions using identical categories;
- identify recurring explanations such as planets, meteors or aircraft;
- detect multiple independent reports of the same event;
- judge the quality of individual witnesses without relying solely on narrative descriptions; and

For Ontario, this was especially significant because Ottawa housed the Defence Research Board, Dominion Observatory and several of the federal departments expected to exchange information. The reporting form was intended to support a national process, but its administrative centre lay in Ontario’s federal institutions.
Why the form saw so little use
The reporting form has sometimes been portrayed as evidence of a secret national UFO investigation. The historical record suggests something much less dramatic. Several factors explain why.
The committee lost confidence in the value of large-scale investigation
As Project Second Storey reviewed available cases between 1952 and 1954, members increasingly concluded that most reports contained too little reliable information to support meaningful scientific analysis. They also found no convincing evidence that the sightings represented a defence threat.
If the underlying reports were generally weak, expanding the reporting system became a lower priority. No department became the permanent owner
The form depended on cooperation between several organisations rather than belonging to a single agency with clear authority.
Reports might originate with police, military personnel, Transport officials or ordinary members of the public. Without one department responsible for enforcing a common procedure, adoption remained inconsistent. Public reporting remained decentralised
Even after Project Second Storey ended, Canadians continued sending reports to whichever organisation they considered appropriate. Some contacted the RCMP, others the Department of Transport, National Defence or local authorities.

What the failure meant for Canadian UFO records
The limited adoption of the reporting form had long-term consequences that historians continue to note.
Canada eventually accumulated thousands of UFO-related records, but they were dispersed across numerous departments with differing procedures and priorities. This fragmentation also complicates comparisons between cases. Some reports contain detailed witness interviews and technical observations, while others preserve little more than brief correspondence. The uneven quality of the archive reflects administrative history as much as the quality of the sightings themselves.
Why the reporting form still matters
The reporting form’s historical importance lies less in how often it was used than in what it reveals about Canadian policy.
Project Second Storey did not assume that every unexplained aerial report deserved a major investigation. Instead, it argued that any serious assessment required consistent, objective observations before conclusions could be drawn.
